We make sure that our patients are confidant and comfortable as they prepare to go home.
When residents are able to recover and thrive under our personalized care, we have done our jobs well. But leaving a secure place like our facility can be difficult; we understand that our patients will need to readjust to their old environment.
In order to facilitate a smooth of a transition as possible, we create individualized discharge plans for each and every one of our patients. We make sure that your loved one will know what therapies they need to maintain their health, as well as addressing medication and dietary concerns. We also plan what steps should be taken in an emergency and who the patient should contact.
We want all of patients to leave feeling confident and secure in their change of environment, and make the move as stress-free as possible.
